During this consultation, we prioritize understanding what matters most to you. We delve into your life history, health status, daily routines, and lifestyle preferences. This information empowers us to collaborate with you in crafting a personalized care plan aimed at supporting your goals and maintaining independence within your familiar surroundings.
By fostering this connection, they become trusted companions, offering support tailored to your needs.
Once we’ve established the agreed-upon visitation schedule and assigned the appropriate Care Professionals, we ensure that all details are accurately recorded in our care software to generate your personalized Schedule.
Our meticulous scheduling guarantees consistency in care provision, ensuring that you or your loved one always anticipate the arrival of a familiar Care Professional. This continuity of care enhances comfort and confidence in the care experience.
Understanding that circumstances can evolve swiftly, once the care package is operational, we conduct routine Quality Assurance visits and Service Reviews.
